Description

JOB SUMMARY: Performs age and developmentally appropriate nursing related tasks and services delegated by a licensed nurse to adult and geriatric patient/resident populations. After appropriate delegation by the supervising nurse, the nursing assistant shall utilize knowledge of resident rights, legal/ethical concepts, communication skills, safety and infection control while performing the following: basic nursing skills, personal care skills, basic restorative skills, mental health and psychosocial skills, communication skills and nursing team member skills. Delivers resident patient care according to the Scope of Service for the Living Center. Adheres to and practices the St. John's Medical Center Corporate Compliance Program and participates in Performance Improvement activities. Fosters a culture of patient/resident safety on the unit. Practices within the scope of an CNA and compiles with standards of practice for long term care facilities 

ESSENTIAL FUNCTIONS:

Resident Care Activities

  • Provides age and developmentally appropriate personal hygiene to residents (peri care, oral care, baths, toileting, changes bed linen, dressing resident) morning, evening and PRN
  • Provides interventions to increase residents quality of life and self worth. Involves resident in decisions affecting their care
  • Encourages self care/independence with ADL's and implements aide interventions as noted on care plan
  • Makes rounds to anticipate needs and answers lights promptly
  • Checks residents oxygen tank and or concentrator for correct liter flow. Makes certain that portable oxygen tanks are full to meet residents needs
  • Follows residents plan of care as outlined by nursing staff and /or daily aide sheet
  • Takes and records vital signs, height and weight, I & O, dietary intake as directed
  • Give baths as scheduled and documents accordingly
  • Assists with care such as positioning, lifting, turning ambulating residents using good body mechanics, using proper equipment and tools to facilitate care
  • Aware of rehabilitation goals for each resident. Implements and documents rehab measures; i.e., ROJM ambulation, splints, speech and OT recommendations
  • Aware of and provides interventions to meet age and developmentally appropriate psychosocial needs of resident. Listens and communicates respectfully to residents. Encourages residents to communicate opinions, needs
  • Prepares residents for meals, distributes food trays, aids or feeds resident as needed - aware of dietary
    rehabilitation needs of all residents. Knows resident diet. Offers snacks and adequate hydration to resident
  • Follows BSI and universal precautions when performing all resident care activities

Indirect Resident Care Activities

  • Handles and cares for resident clothing and other personal property (including prosthetic devices, hearing aids, glasses, dentures, contact lenses, etc.) from admission to discharge
  • Labels resident's personal belongings with identification. Assists in keeping resident's room neat, safe and clean
  • Reports any changes observed in condition or behavior of resident. Documents in graphic records and Resident Care Flow Chart accurately
  • Assists in maintaining safe and clean environment by reporting malfunctioning equipment, cleans equipment as assigned, keeps hallways clear, reports other problems to management as needed. Implements fire and disaster plan correctly Practices good BSI
  • Performs fundamentals of secretarial duties (i.e., answers phones, paging, relaying messages.) as needed
  • Maintains, develops and updates skills to implement ideal care and service.

JOB REQUIREMENTS:

Required Education & Experience: Wyoming CNA required at time of hire and upon expiration of credentials. BLS required at time of hire and upon expiration of credentials.

Preferred Education & Experience: Good communication, good interpersonal and customer service skills. Must be able to work in a team situation and be able to handle stressful situations.

FUNCTIONAL DEMANDS:

Physical Requirements: Employee must be physically active including heavy lifting and moving of residents. Must possess sight/hearing abilities to perform job requirements. Must possess the ability to push, pull, bend and move patients/residents, equipment, and supplies. Must be able to lift up to 50 pounds. Employee may be required to take call shifts and must possess the ability to do shift work. 75% of shift spent standing and walking. Intermittent sitting and computer use.
