Description
Job Summary:
Responsible for managing and providing a robust and stimulating Life Enrichment Program for the residents of Sage Living. Assesses and evaluates the recreational needs of adult and geriatric residents. Plans, organizes, facilitates, and adapts age and developmentally appropriate life enriching activities for this population of residents. LE activities should promote health, maximize resident potential and independence, and meet the individual needs and interests of each resident through meaningful activity. Overall, this position is responsible for enhancing the quality of life for, and bringing joy to, each resident. Delivers services according to the Scope of Service for Sage Living. Adheres to and practices the St. John’s Health Corporate Compliance Program and participates in Performance Improvement activities. 

Essential Functions:
Directs the Life Enrichment Program
• Completes recreation and minimum data assessments of residents on admission and ongoing as necessary
• Develops individualized, age and developmentally appropriated LE recreation plans
• Designs and implements a comprehensive activity program that provides for a variety of age and developmental groups as well one to one interaction, presented to residents in a manner that facilitates their involvement
• Evaluates the overall LE program targeting an active, varied, community connected, age-appropriate program, as well as one that meets the specific interests and needs of each resident
• Collaborates with, supervises, and evaluates the Life Enrichment Program staff
• Attends and participates in resident care conferences to exchange information pertaining to residents which will assist with their care and support by the caregiving team
• Documents relevant information regarding residents’ recreation plans; their engagement in LE opportunities and changes to the plan when it’s not meeting the resident’s needs
• Communicates with residents and team members regarding scheduled LE activities, upcoming events and special programs
• Plans for quality assurance and quality improvement to ensure that problems are identified promptly and resolved
• Plans and coordinates community outings. Is responsible for the safety of all participants. Is responsible for managing/coordinating the maintenance of the Sage Living bus. Oversees the bus wheelchair lift and tie down systems.
• Orients, trains, and supervises the volunteers who work in Sage Living.

Interdepartmental and Community Liaison
• Facilitates monthly Resident Council meetings
• Serves at the liaison between St. John’s Health Auxiliary and Sage Living 
• Communicates with the hospital Volunteer Coordinator regarding volunteer needs at Sage Living
• Collaborates with the Music Therapist for both weekly LE activities and special events
• Coordinates with community members and groups (e.g. school group, churches, musicians, clubs, etc.) for LE opportunities that support social interaction, entertainment and community connections
• Collaborates with other Sage team members for the monthly Sage Advisor newsletter; gathers information, writes articles as needed

Budgeting and Cost Awareness
• Manages the cost of running a robust LE program, taking into consideration seasonal activities, volunteers, donations, etc. and provides activities within budget
• Manages the Life Enrichment Activity Fund

Resident Care Activities
• Orients and trains staff about LE programs and staff responsibilities
• Works to utilize Eden Alternative principles in all aspects of the Sage Living environment and activities, including training staff and promoting the Eden philosophy
• Understands and complies with CMS regulations
• Shops weekly for LE, Sage Living, and residents’ personal needs and requests
• Participates, as needed, in the care of the bird aviary and other Sage Living pets 

Job Requirements:
Minimum Education
Preferred: Certified Therapeutic Recreation Specialist (CTRS)

Minimum Work Experience
Required: Ability to plan, organize and implement independent, one-on-one and group activities. Good communication and leadership skills required.
Preferred: Experience working with residents of a nursing home, assisted living, or other residential care facility. Formal training, education and activity planning.

Required Licenses: BLS required at time of hire. Certified Activities Professional or Certified Therapeutic Recreation Specialist and Valid Commercial Driver's License (CDL) preferred.

Functional Demands:
Working Conditions: 
Long term care setting. Must be on-site at the SJH campus or other locations to fulfill the responsibilities of this role. Work environment includes offsite errands and excursions. Occasional exposure to inclement weather including heat and cold, frequent driving required.

Physical Requirements: Frequent physical activity for up to 8 hours at a time. Physical tasks may involve walking, sitting, standing, driving, reaching, bending, squatting, pushing, and pulling. Must be able to perform mobility assistance tasks including pushing a wheelchair on even or uneven terrain, assisting residents to stand from a seated position, and properly utilizing accessible bus tie-down system. Must be able to lift at least 30 pounds.
