Description
Job Summary
Provides psychosocial support to patients, families and significant others facing the impact of potential or actual diagnosis of cancer. Services include advising family care givers, providing education and counseling, and making referrals for other services. May also provide care and case management or interventions designed to promote health, prevent disease, and address barriers to access healthcare. 

Essential Functions:
Patient Assessment/Plan of Care
· Conducts a comprehensive patient assessment and collaborates with the patient, medical oncologist, PCP and other members of the health care team to develop and individualized plan of care with targeted interventions. 
· Continually monitors patient/family response to plan of care and revises as indicated. 
· Assess over time the health care, educational and psychosocial needs of the patient and family. Using standardized assessment tools such as distress screening, functionality and health risk assessments. 
· Coordinate patient care through ongoing collaboration with medical oncologist, PCP, patient/family, community and other members of the health care team. 
· Fosters a team approach and includes patient/family as active team members. 

Assessment/Plan of Care Follow-up
· Provide follow-up with patient/family when patient transitions from one setting to another. 
· Completes timely post clinic/hospital follow-up: review discharge instructions, coordination of care, follow-up appointments and problem solve barriers. 
· Organize patient and care giver support groups.
· Counsel patient family members to assist them in understanding, dealing with, and supporting the patient. 
· Counsel individuals and discuss the emotional challenges of dealing with a potentially life-limiting and make referrals to mental health professionals as indicated. 

Survivorship
· Work with the nurse practitioner and nursing staff to develop and create survivorship care plans. 
· Help patients and survivors create a "road map" with tangible tasks so that the individual with cancer can learn live with chronic illness. 
· Educate patients and families on, and assists in, preparation of advanced directives. 
· Assistance with transitioning to hospice and/or bereavement counseling. 

Other Duties
·Cross trains in other departments of the hospital to cover social work needs when required.

Job Requirements:
Minimum Education:
    Required   
Masters degree in Social Work
                   
Minimum Work Experience:
    Required    
Previous experience in an acute care or outpatient setting with specific experience in discharge planning, clinical therapy skills and in-service work. Good phone etiquette, excellent communication and customer service skills. Basic computer skills. 

    Preferred   Experience working with Oncology patients

Required Licenses: [Wyoming, United States] Social Worker. Wyoming SW.

Functional Demands:
Working Conditions:
Hospital proper with occasional errands, meeting, etc. in the community. Frequent stressful situations. Must be on-site at the SJH campus or other facilities to fulfill the responsibility of this role.

Physical Requirements: Stooping/bending, finger dexterity, seeing, hearing, speaking, lifting usual weight of 10 to 25 pounds. Frequent standing, walking, reaching at and below shoulder level. Occasional sitting, kneeling, climbing stairs, walking, squatting, pushing/pulling, reaching above shoulder level. Operates computer, calculator, copier, facsimile and telephone.
