JOB SUMMARY

Provides and coordinates age and developmentally appropriate care of adult and geriatric residents who are admitted to the Living Center for long-term or skilled services. Delivers this care using the nursing process and according to the Scope of Service for the Living Center. Adheres to and practices the St. John's Medical Center Corporate Compliance Program and participates in Performance Improvement activities. If LPN,  Practices under the supervision of a Registered Nurse.

DUTIES

Delivers care to long-term and skilled residents, following each resident's personalized plan of care, with goals of restoration of functions where possible and goals of supportive care to prevent further deterioration when restoration is not possible.

Monitors resident status and initiates appropriate interventions as care-planned, in regard to resident skin condition, mobility status, sensory impairments, pain status, cardiovascular status, respiratory status, renal status, nutritional status, reaction to medications, psychosocial status and cognitive status. Reports all
changes in condition to supervising RN, and to resident families and MD as appropriate. Documents all care provided accurately and timely. Processes orders from care providers.

Provides direction, supervision and coaching to Certified Nursing Assistants, Nursing Assistants, Restorative Aides and/or Resident Care Aide in coordinating and documenting their care for residents.

Assures all pertinent resident status information is communicated to oncoming nurse via change of shift report.

Assumes the following responsibilities in supervisor's absence: secures staffing, documents events for further evaluation, refers problems to supervisor.

JOB REQUIREMENTS:

Minimum Education: IV certified in WY preferred

Minimum Experience: Experience in LTC/skilled care preferred

Required Licenses: current license as RN or LPN in WY, current BLS

WORK ENVIRONMENT:

Long-term care clinical setting, occasional stressful situations, irregular hours, care of patient/residents with complicated medical/psychosocial conditions. Night shifts, weekends, holidays, 12 hour shifts

PHYSICAL DEMANDS:

Employee must be physically active including heavy lifting and moving residents, equipment, and supplies. Must possess sight/hearing abilities to perform job requirements. Must possess the ability to push, pull, bend and reach. Must be able to lift up to 50 pounds. Employee may be required to take call shifts and must possess the ability to do shift work. 75% of shift spent standing and walking. Intermittent sitting and computer use.
