Description
Job Summary
Responsible for providing care services, including assessing, diagnosing, prescribing, treating and educating patients. Works collaboratively with physicians, physician assistants, nurse practitioners, and other clinicians in a team approach to patient care. Provides family practice services to patients already established with the practice and creates access for additional patients to receive effective, efficient, and safe care.

Essential Functions

Patient Care
  • Assesses patient status by obtaining health history through patient/family interviews and chart reviews; assessing presenting illness, risk factors, family history, psychosocial situation, and cultural factors; and performing appropriate physical examination and plan of care. Performs appropriate medical procedures within scope of practice.
  • Orders/performs appropriate laboratory and radiology diagnostics as well as other screening tests. Interprets these tests. Seeks other information as needed, including consultation with physicians and other clinicians, for evaluation of illness. Integrates data to determine diagnosis and therapeutic plan, including identification of any health risks.
  • Develops and implements treatment plan by prescribing/dispensing medications and/or injections in compliance with medical practice guidelines and state law.
  • Delegates direct/indirect patient care to appropriate personnel.
Patient/Family Education
  • Instructs patients/families regarding medications and treatments. Educates patients regarding health promotion/illness prevention. Recommends appropriate community resources to meet patient/family needs.
  • Communicates appropriate case management information to other professionals and community agencies. Acts as consultant/collaborator to clinical team on area of specialty. Collaborates with physicians and other clinicians to ensure continuity of care between visits according to treatment plan including triaging patient calls/emails.
  • Provides excellent customer service to internal and external customers. Provides knowledge, timely and appropriate information to customer/patients and staff in a friendly manner. Maintains positive peer relationships and performs as a team player. Demonstrates respect and sensitivity to cultural and social differences in interactions with others.
Documentation/Education
  • Prepares documentation for medical records including updating patient medical chart by posting examination and test results, diagnosis, medications, and treatment in written/computerized manner in a timely fashion.
  • Participates in peer reviews, chart reviews, staff education, clinical guidelines development, and other continuing education and quality assurance activities to demonstrate compliance standards, regulation, policies and procedures.
  • Complies with patient confidentiality requirements and HIPAA. Promotes advocacy. Understands and follows state and federally mandated requirements regarding patient confidentiality and patient rights.
  • Adheres to all relevant policies, procedures and practice guidelines.
Knowledge
  • Knowledge of advanced practice including anatomy and physiology, pathophysiology, pharmacology and therapeutics, basic nutrition, behavioral factors, psychosocial/family systems, diagnostic testing, interpretation of results and clinical decision making, alternative treatment modalities.
  • Knowledge of current practices and regulations related to evaluating and providing patient care and professional ethics related to the delivery of medical care.
  • Knowledge of medical equipment and instruments used to administer patient care.
  • Knowledge of common safety hazards and precautions to establish/maintain a safe work environment.
  • Knowledge of community, professional and educational resources.
Job Requirements
Minimum Education
Required: 
May be a graduate of either an accredited school of nursing with advanced preparation as a Nurse Practitioner or a physician assistant education program approved by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA).

Minimum Experience
Required: 
NP must have a minimum of three years of professional nursing experience as a registered nurse. PA must have a minimum of one year of physician assistant experience.

Required Licenses:
NP - Wyo RN with State of Wyoming certification as a Nurse Practitioner with prescriptive authority. 
PA - State of Wyoming license and passing Physician Assistant National Certifying Examination (PANCE).
Current DEA and controlled substance license, current BLS, ACLS, and PALS.

Functional Demands

Working Conditions: 
Hospital based outpatient department, frequent interruptions, sometimes stressful environment. Combination of exam rooms and medical offices. Frequent exposure to communicable diseases, toxic substances, ionizing radiation, medicinal preparations and other conditions common to a clinic environment. Must be on-site at the SJH campus or other facilities to fulfill the responsibilities of this role.

Physical Requirements:  Standing and walking for extensive periods of time. Occasional lifting and carrying items weighing up to 10 pounds. Correct vision and hearing within normal range with or without use of corrective devices. Must be able to verbally interact with staff, patients and the public. Manual dexterity for writing and computer use. Requires working in emergency situations and may involve irregular work hours including weekends, evenings, holidays and natural disasters.
